care services to you properly, we must

Care and Support Packages you can take advantage
of a range of services brought to you in your
own home. The aim of supported living is to give
individuals the opportunity to live
independently in their own home, making their
own choices and leading their life in the way
they want to.
Our Care Manager will plan the
Care Package, based on individual needs, and
then devise the structure and times for how and
when a Service User want to be supported.
We work closely with the people
we support, those within their social network
and with our colleagues in health and social
care through an open and transparent approach
with clear and ongoing communication.
