The aim of supported living is to give individuals the opportunity to live independently in their own home, making their own choices and leading their life in the way they want to.

Our Care Manager will plan the Care Package, based on individual needs, and then devise the structure and times for how and when a Service User want to be supported.

We work closely with the people we support, those within their social network and with our colleagues in health and social care through an open and transparent approach with clear and ongoing communication.

Some of the key choices people will be able to make are:

  • What support do I want?
  • How many hours a day?
  • Who do I want to support me and how?
  • How I want to spend my time?
What is the budget for my service - have I got an Individual Budget or Direct Payments, how will my care manager or funding authority work with me?
